Coronoirus - COVID-19 - The NHS and Public Health organisations are extremely well prepared for outbreaks of new infectious diseases. The NHS has put in place measures to ensure the safety of all patients and NHS staff while also ensuring services are available to the public as normal. Try the NHS App

If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet.

You can use the NHS App to check your symptoms and get instant advice, book appointments, order repeat prescriptions, view your GP medical record and more.

If you already use SystmOnline you can continue to use it. You can use the NHS App as well.

Returning travellers

Stay indoors and avoid contact with other people immediately if you’ve travelled to the UK from:

  • Hubei province in China in the last 14 days, even if you do not have symptoms
  • Iran, lockdown areas in northern Italy or special care zones in South Korea since 19 February, even if you do not have symptoms
  • other parts of mainland China or South Korea, Hong Kong, Japan, Macau, Malaysia, Singapore, Taiwan or Thailand in the last 14 days and have a cough, high temperature or shortness of breath (even if your symptoms are mild)
  • other parts of northern Italy (anywhere north of Pisa, Florence and Rimini), Cambodia, Laos, Myanmar or Vietnam since 19 February and have a cough, high temperature or shortness of breath (even if your symptoms are mild)

Do not go to a GP surgery, community pharmacy or hospital. Use the 111 online coronavirus service to find out what to do next.

Since 2015 the NHS has asked that each practice updates patients records to ensure you have a named accountable/usual GP.  We have done this.  This does not prevent you from seeing any GP.   If you would like to know who your accountable/usual GP is, please ask when next in surgery.

If your GP decides that it is necessary to refer you to see a specialist at hospital, your referral will be usually be sent via "e-Referrals", our electronic referrals system.  Where possible, we will send you a choice of available healthcare providers, with instructions on how to book your appointment. 

Some secondary care services are triaged by a consultant.  If your referral has gone to a triaged service, you will be contacted by the service provider to arrange an appointment.

Hearing Test – Holistic Hearing offer NHS hearing tests at Collingham Medical Centre. Patient registered at Collingham can self-refer by contacting our PCA team to book an appointment.  

 

(If you wish to have a hearing test at the hospital, this does require a doctor’s referral.)

 

 

Physiotherapy – Print off a self-referral form or pick one up a from the reception desk and post directly to the Physiotherapy Department.

Vasectomy - Patients can self-refer to the Nottingham Road Clinic by calling 01623 624137 or emailing nottinghamroad.clinic@nhs.net to book an appointment (no referral form).
