A Carer is anyone who cares, unpaid, for a friend or family member who due to illness, disability, a mental health problem or addiction cannot cope without their support.

You are a Young Carer if you do this role and are under 18 years of age.

If you are looking after someone, we would like you to let us know so we can provide the support and information you might need.

You can be added to our Practice Carers Register and we can then give you further information and support.

A Carers Champion is a Staff member at the Practice who supports Carers.

Change Grow Live – Alcohol Service – Patients are able to self-refer to service and they provide support inpatient detox and prescribinghttps://www.changegrowlive.org/

First Contact Physiotherapist – delivers services to 16+ year old patients with musculoskeletal problems (neck, back, bones, joints or muscles). The FCP is a physiotherapist with specialist training who is able to see patients with these conditions, deal with related medication and organise services or investigations if appropriate.  Self referral to to this service is available.

Physiotherapy – Print off a self-referral form or pick one up a from the reception desk and post directly to the Physiotherapy Department. 

Vasectomy – Patients can self-refer to the Nottingham Road Clinic by calling 01623 624137 or emailing nottinghamroad.clinic@nhs.net to book an appointment (no referral form).

e-Referral

e-Referral is a service that allows you to choose your hospital or clinic and book an appointment with a specialist.  When you and your GP agree that you need to see a specialist, you will be able to choose from a number of service providers the five closet hospitals are:  

  • Newark General Hospital
  • Kings Mill Hospital
  • Queens Medical Centre
  • Nottingham City Hospital
  • Lincoln County Hospital

The Benefits of e-Referral

  • You can also choose the date and time of your appointment.
  • You experience greater convenience and certainty. With the old system, your hospital or clinic chose the date and time of your appointment. This may not have been convenient for you. With e-Referral, the choice is yours.
  • There is less chance that information will get lost in the post as information is shared electronically.  

Booking your Appointment

Once your referral has been processed by the practice, we will send the booking information to you.  You can choose your preferred healthcare provider and book an appointment by telephone or online.

Private Services

Outside the scope of our NHS contract, we do support our patients with some private services. We do use the information stored in your medical record (with your consent) to undertake this, for example 

  • The administration of some travel vaccinations not available from the NHS
  • Medical examinations, ie HGV, employment or sports applications
  • Medical reports to insurance companies to support insurance policy applications/claims
  • Letters of support or letter of fitness to participate

These services are all non-NHS services and are chargeable.  The time the team need to invest in making this happen for you needs to be funded.  A note of the fees can be found on our website and in the waiting area of the Collingham Medical Centre.

If you have an appointment for a medical examinations and cancel, or DNA, within 24 hours of this appointment date/time, the appointment will still be chargeable and an invoice will be forwarded for payment.  A further appointment would be made, if required, once payment received.

Travel Vaccinations

A range of immunisations are available for travel.  A travel form is required to be completed, on receipt of the completed form a nurse will review your records in line with your destination/activities.  The nurse will write to you with the recommendations for you to consider prior to travel.  Advice and some immunisations are provided by the NHS.  Some immunisations are only available privately.  If you decide to take advice, please make an appointment for your immunisations.  Please allow a minimum of 6 weeks before travel when handing in your completed form.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Long Term Conditions and Medication Reviews

The need to improve the treatment and management of long-term conditions is the most important challenge facing the NHS. Improving care for people with long-term conditions must involve a shift away from a reactive, disease-focused, fragmented model of care towards one that is more proactive, holistic and preventive, in which people with long-term conditions are encouraged to play a central role in managing their own care.

In response to this, we use a system for calling in our patients for annual reviews of their long term conditions. The system will group all your appointments into one appointment with the Healthcare Assistant for different tests and examinations. The results will then be sent to your GP for review, should the GP feel you need to see either a Specialist Nurse or themselves this will then be arranged. You may not require to always be required to come in for a second appointment as your condition may be stable/well controlled.

You will be called each year the month of your date of birth. The appointment times will vary dependant on your long term condition.

Should you not book an appointment after your first invitation you will be sent another invite one month later.  Should you not respond to this, we will assume you do not wish attend this appointment and we may make no further contact until the following year.

Patients that will be invited for annual review

Any patients with a diagnosis of one or more of the following conditions will be invited to attend for an appointment:

As part of your annual review you may be asked to do some home blood pressure monitoring prior to your appointment.  Click the link below to open a Home Blood Pressure Monitoring Diary to record your results.  If you are unable to print this form, please write down your results in a similar format.

You should record your blood pressure twice a day, morning and evening, for 1 week, and bring the results to your appointment.

Additional Professional Services

  • Physiotherapy – Thursday (clinic times vary).  Self referral to this service is available.
  • Clinical Psychology and Counselling – We have a clinical psychologist and counsellor in the practice. Please see your doctor about this first. Referral to this service is by doctor only. 
  • Minor Surgery – Some minor procedures are carried out here with the nurse assisting the doctor in clinic  Your doctor will advise you if an appointment is needed.

Nursing Services

The nursing team offer the following services:

  • Family Planning – By doctors and the midwife.
  • Early Pregnancy – With the midwife at 8+ weeks.
  • Antenatal Care – Monday afternoons.
  • Blood Tests
  • Infant and Adult Immunisations
  • Cervical Smears
  • Dressings and Removal of Stitches
  • Ear Irrigation 
  • Monitoring Blood Pressure, Asthma, Hypertension and Diabetes.
  • Minor Illness – Our nurse team are available to see patients with minor conditions such as urinary tract infections, sore throats, conjunctivitis, etc. Please note these sessions are by same day appointment.
  • NHS Health Checks – These are offered, by invitation, to patients aged between 40 and 74 once every 5 years in line with NHS guidance.  They offer simple lifestyle checks such as weight, blood pressure and urine test.
  • Well Person Checks – These are available to all patients. Please make an appointment at afternoon or evening nurses’ clinic. These offer simple checks such as weight, blood pressure and urine tests, plus cancer smears for women.

  • Minor Injury – If a minor injury requires suturing, glue or steri-strips our same day nurse may be able to help.
  • Coils & Implants – provision of IUD fitting, monitoring and removal of all types.  This service requires a referral by your GP.
  • Same Day Telephone Triage Appointments – We offer a Same Day Telephone Triage Appointment system  for patients who need to be seen for urgent clinical needs.  We ask you to remain close to your telephone as the doctor may only be able to attempt to call you twice.
