If you move into the practice area you can register as a patient at Collingham Medical Centre using the link below. Should you need to see a healthcare professional, you will be able to whilst your registration details are being processed.
If you take regular medications, we recommend that you request a month’s supply from your previous GP practice before completing our online registration form. This is to allow plenty of time for the transfer of your prescription.
If you are over the age of 16 and have not been seen for the past 3 years, you may request a health check. We strongly encourage you to do this as all adults should have their blood pressure checked every 3 years.
If you are over the age of 75 and have not been seen for the past 12 months, you may request a consultation. We will be happy to see you.

Know your NHS number
NHS England has an online checker to enable you to find your 10-digit NHS number. Simply, visit the online tool (link below) and enter your details to validate your identity to obtain your personal NHS number.
Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

If you would like to be registered with us as a carer or a patient with a carer, please let a doctor, nurse or PCA know.
